靈實翠林長者日間訓練中心 EN

中心簡介


由護士、註冊社工、職業/物理治療師、健康服務員組成跨專業團隊，以一站式個案管理，為長者提供多元化的訓練。長者除可在寬敞及設備齊全的日間中心接受服務外，亦可安排各項家居照顧，包括個人護理、復康及認知訓練、短暫住宿服務及點對點專車接送等。以貼心的服務讓長者保持生活質素，安享晚年。另外，社工亦會為長者及家人提供輔導，紓緩照顧壓力。



服務對象

  • 經社會福利署安老服務統一評估機制評定為身體機能中度或嚴重缺損
  • 正在長期護理服務中央輪候冊輪候資助社區照顧服務及或住宿照顧服務
  • 目前未接受任何住宿照顧服務或資助社區照顧服務
  • 已持有社會福利署所簽發的電子社區券的長者
  • 未持有服務券的長者可使用中心的自負盈虧服務



服務內容


基本護理

定期提供健康檢查：量度生命表徵（血型、體温等)，有需要時提供血糖檢驗，藥物指導、傷口護理等。

膳食供應

中心會供應均衡營養之早餐、午餐、茶點。

個人照顧

中心會提供日常起居及個人生活照顧、餵食及如廁訓練等。

復康運動

由物理治療師及職業治療師為個別長者訂定適合的復康運動及訓練，並在指導下使用中心的運動設施。

家居服務

由個案經理與毎位長者商討並落實「個人照顧計劃」，並會根據所選定的服務組合來提供上述服務。
