日間復康及記憶護理中心

我們的長者日間中心，可在短時間內為有需要的長者提供日間照顧服務，藉此舒緩照顧者的日間照顧壓力，並讓體弱長者走出家門，在日間中心接受復康及護理服務，以及擁有自己的社交生活。

護理及復康需要評估

由社工、護士及物理治療師或職業治療師評估長者的身體狀況，度身訂造適合長者個人的復康及照顧計劃。

復康巴士接送往返中心

按需要提供點對點復康巴士接送長者，家人只須在居所附近的上落車地點接送 (名額有限，須向中心查詢)

按需要提供點對基本護理

為長者量度各項生命表徵，有需要時提供血糖檢驗、傷口護理、藥物指導、營養指導等，確保長者的身體狀況安全穏定。點復康巴士接送長者，家人只須在居所附近的上落車地點接送 (名額有限，須向中心查詢)

個人照顧

由嚴格受訓的工作人員負責長者的個人活動、餵食、服藥、如廁等，讓長者感受到細心的照料和關顧。

復康運動

由物理治療師或職業治療師為長者訂立個人化的復康運動，並在職員指導下使用復康訓練設施，以改善身體機能。

認知訓練活動

透過針對性的小組活動或遊戲，訓練長者的記憶力、溝通表達、現實導向及生活技能等，以延緩長者的認知退化情況。

社交活動及心靈關顧

透過多元有趣的社交康樂活動，以及個別關顧長者的身心狀況，讓長者保持穩定愉悅的情緒。

膳食服務

供應營養均衡、適合長者的早、午餐及下午茶餐，並按長者的吞嚥狀況而特別提供碎餐或糊餐，以及個別餵食。

陪診

如有需要，可另行安排陪診服務，由陪診員護送長者到指定診所或醫院求診或覆診，並跟進取藥等程序。

我們的長者日間中心同時亦是社會福利署認可的「長者社區照顧服務券計劃」的服務提供者，持券者可用社區券面值支付服務費用，未持有服務券的長者則可自費使用服務。詳情請向以下單位查詢：

Basic provisions

In accordance with the Personal Data (Privacy) Ordinance, Haven of Hope Christian Association (Hainsteel) and/or its service units adopt the following policies to ensure that the personal information of web users is kept confidential.
