長者社區照顧服務券計劃

「長者社區照顧服務券計劃」專為合資格的持券長者而設，條件如下：

  1. 在社署「安老服務統一評估機制」下獲評定為適合社區照顧服務或院舍照顧服務
  2. 正在「資助長期護理服務中央輪候冊」輪候資助社區照顧服務及／或院舍住宿照顧服務
  3. 目前尚未接受任何院舍住宿照顧服務或資助社區照顧服務

服務組合

長者可按需要同時使用中心為本的「日間中心服務」及/或 家居為本的「上門照顧服務」

收費模式

根據社會福利署制訂的標準而釐訂服務收費，並由社署根據長者及同住家人的收入進行經濟狀況審查，以釐定長者需負的付款級別 (即服務組合金額的5%、8%、12%、16%、25% 或40%)，餘額則由社署負責支付。

Basic provisions

In accordance with the Personal Data (Privacy) Ordinance, Haven of Hope Christian Association (Hainsteel) and/or its service units adopt the following policies to ensure that the personal information of web users is kept confidential. Personal data including but not limited to name, telephone number, fax, address, email address and donation record will be used for fundraising, organizing and organizing activities to support, respond to, publicize and promote our charity and/or health promotion work. Our hospital will never disclose any personal information to outsiders without the consent of the website user.
