靈實樂華長者日間中心EN

中心簡介


由護士、註冊社工、職業/物理治療師、健康服務員組成跨專業團隊，以一站式個案管理，為長者提供多元化的訓練。長者除可在寬敞及設備齊全的日間中心接受服務外，亦可安排各項家居照顧，包括個人護理、復康及認知訓練、短暫住宿服務及點對點專車接送等。以貼心的服務讓長者保持生活質素，安享晚年。另外，社工亦會為長者及家人提供輔導，紓緩照顧壓 力。




服務對象

  • 經社會福利署安老服務統一評估機制評定為身體機能中度或嚴重缺損 
  • 正在長期護理服務中央輪候冊輪候資助社區照顧服務及或住宿照顧服務 
  • 目前未接受任何住宿照顧服務或資助社區照顧服務 
  • 已持有社會福利署所簽發的電子社區券的長者 



服務內容


本中心提供 中心為本 及 家居為本 服務，包括復康運動、護理服務、個人照顧、住宿暫託、日間暫托等

* 由個案經理與毎位長者商討並落實「個人照顧計劃」，並會根據所選定的服務組合來提供上述服務。
