由物理治療師負責提供復康評估和治療服務，為長者制定個人化的復康和物理治療計劃，以在熟悉的環境中接受專業的治療和訓練，幫助長者改善身體狀況、增強功能恢復、延緩機能衰退並提高生活質量。
物理治療師會根據長者的實際情況制定復康運動訓練方案，可以由物理治療助理、保健員或起居照顧員協助長者進行，再由物理治療師定期評估跟進，並按進度調整訓練方案。
